Oral Medications form Must be signed by Doctor!!

Dear Parents and students going to DC,


 If you have not had your Oral Medications authorization Form signed by a doctor please get this completed before we leave tomorrow.

One more thing-
If a student has a chaperone going on the trip, they do NOT need to do the forms.

Also, some of you already did get a signature anyway- you don’t need to do it again.

Hello everyone,

I just met with our school nurse, and she let me know of our school policy that we DO indeed need to have doctor’s signature on the medical forms, *EVEN FOR OVER-THE-COUNTER MEDICINES* such as Tylenol, Ibuprofen, Claritin, etc. I am extremely sorry; I knew we were in a time crunch and didn’t realize this requirement for over-the-counter as well.

So, she gave us a couple of options.
1)      Have your student not bring what they might not need. So if they are taking something just as a precaution or “just in case,” an option is to just trust that they won’t need it.

2)      Get a signature from your doctor / doctor’s office- this goes for all prescription medicines NOT already on file here at school, and for any over-the-counter medicines. I realize that this is an extremely busy time for all right now, and I truly, truly apologize for giving you one more thing to get done before we leave tomorrow night. However, if you have time (or your student) to zip by your doctor’s office, any licensed medical health professional in your doctor’s office can sign the form, our nurse says. You don’t need to make an appointment, it just needs to be signed.

I am attaching the form because we don’t see the majority of our students today in class, and many have diligently turned in their forms already.

Dear Choristers and Parents,

We have our Contest Music Concert Coming up on Tuesday March 13th. at 7pm in the forum. Those of you who have been checking the calendar may have noticed that this concert was on the 16th on the google calendar on this blog. I have just caught that mistake and changed it to the correct date. The School calendar is and has been correct.

For Tuesday night please make sure that...

-Choristers arrive by 6:00 Dressed and ready to warm up.
-Mixed choir must wear any combination of Black and white. No tshirts or holy jeans please wear dress shoes but no Super high heals!
-Concert choir and Choraliers please wear Black on the bottom and black dress shoes.
-Storm singers are in their outfits that are clean and pressed.

Remember that concert etiquette is an important part of our concerts at SQHS. please follow these guidelines.

  • Arrive on time. 
  • Don't get up and leave or come in to the auditorium during performances. please wait until the applause to move in the auditorium.
  • Do not have conversations during performances.
  • interruptions or distractions by digital devices are not permitted. Just turn it off. we want you to listen to us and not worry about your digital world.
  • young children are welcome.
We have worked hard on the music that we will be performing and it carry's a powerful message that is different in every piece. please be attentive and in the moment with us.
